It's neither. B is more accurate. Left lane turns into left lane. Right lane turns into middle lane. Then you change lanes once turned if you need to get into a different one.
No, you are allowed to go straight into the outer lane if its free.
There is something similar near my place. I'm not sure which is right, but I know lots of others would be e confused too. So if I'm on the left, I stick leftist, and if I'm right I stick to my most right. And then if I want to be in the middle, I change lanes afterwards. It just seems safer and more certain for all parties involved.
Nothing worse than being right, having a crash, and just arguing it with the other party.

I think B is right, but I do what cloudy does too. I've seen cars rushing into the middle lane from both lanes before (thankfully not at the same time). From what I've seen. it appears that people would follow the leading cars as well. The first car from either lane to head to the middle sets the precedent. Cars in the other lane just sticks to theirs and merges later.

Even with lane markings there are some bloody retarded people that honk you when they stray from their left lane.
It is B. The outer lane is meant for right turn. If you wanted to turn right after turning left, then you can go directly to the right lane when safe to do so.
