Clowns are quite scary to some. I'm sure some kids will have a phobia of clowns. A dog petting/therapy session sounds so much better
They call themselves Clown Doctors but don't wear makeup or wigs - just brightly colored "doctors" uniforms. I think they realise that, as you correctly pointed out, some kids might be frightened by actual clowns.

I'm sure you can come with a silly photo that is socially acceptable to 99% of employers so that you don't loose(sic) your job. It's not like they are asking you to pose naked or plank off a balcony. Even a silly grin would probably be good enough.
And it's not income to you, it's a donation from CBA to the charity.
