He can book all that stuff in Cairns, there are shops in the main street for the companies. He would also be highly advised to do the skyrail, particularly the open cage one.
Hot air ballooning and skydiving can be done in the one day easily, it is a bit of a trip out to the reef from Cairns so two days is fine. I would recommend he stay for three though and do the skyrail.
edit make it four, there is a day trip to go white water rafting, the tully one, it's not the one near the town, further away, but it's also a great day out. This is the company I used. http://www.ragingthunder.com.au/

Probably want to spend at least 4 days in Cairns. It's a massive tourist hub and there are so many different activities to choose from. Plus if you only go for two days and the weather is bad for those two days it would be a bit disappointing.

No need to join any long organised tour, other than perhaps booking a day tour. There are so many different reef and day tour operators in Cairns and they will either pick you up from the hotel or they will leave from the middle of cairns. The Hot Air Balloon and the Skydiving companies will probably pick you up from the hotel too, but just confirm with them.

If you want to go up to Port Douglas or down to Atherton Tablelands you can do a DIY tour and just hire a car for a day.

I found that some of the cheapest rates for tours were on bookme… https://www.bookme.com.au/bookings/cairns-port-douglas/home
