I like that in the first question, 'how often do you visit?' you can select 'never' as your answer and then just continue with the questions.

There are many of these for different shopping centers (owned by Vicinity chain), but if you've filled out the one from the OP's link, then you cant do any of the other shopping centers as its one per person
