  Does anyone have the link to the form for existing members?

  The link I've got doesn't allow me to enter my email: "email is already subscribed to list Dr Martens"

  In the T&C's:

  iii. Existing members can enter by completing an online form sent via an email to users subscribed to Dr. Martens during the campaign period.

  But, I do not have any email with an online form?
