For writing what you want engraved it's a little messy but on checkout in the Order Notes section just write what message you want engraved.
Pretty much what ever length you want, although ideally no more than 22 characters per line 4 lines per bottle (except Sinatra and gold is 3 lines)
Just keep in mind the more you write the small the font and the worse it'll look on the bottle.

You can get pretty much any message engraved, I personally got the batman logo done with ascii turned out alright.

    If anyone is looking for this, Costco have wording for free, just have to buy the bottle. got it done on the weekend. Gentleman Jack 1L was $65

      Fair enough, but it's an alternative option for those who a) don't want to go to Costco and b) I'm guessing Costco has it's limits. I'll gladly attempt to break those limits

      And if it's the same as the comment above we have a bigger selection for people too.
