To enter you need to have owned (or been a beneficiary of) one of the following insurance policies in the last 12 months:

• Life Insurance
• Total and Permanent Disability (TPD) Insurance
• Trauma Insurance
• Income Protection Insurance, or
• Funeral Insurance

Follow the competition link to the online survey to rate aspects of your insurer's service and to provide a comment (of 10 words or more) regarding your overall experience and whether you would recommend this insurer to others.

Policies which are held through your super fund are acceptable (you can usually find out your insurer's name on the super fund website).

The winner of the Apple Watch will be randomly drawn from all the entries received.

The winner will need to provide evidence that they have owned, or been a beneficiary of, the policy reviewed and photo ID in order to claim the prize.
