Anyone know of a decent Online COVID Declaration Form??

I'm trying to find something to implement at our factory. We want people to have to declare each day when they walk in. It should be fairly simple - scan a QR, goto a website, add name, phone, email, and say that you agree to the terms, confirm you aren't sick or near someone who is sick.

The NSW government one actually isn't a declaration, it's just an I AM HERE in essence.

If i build one from Google Forms, it doesn't actually take specific action if you select results which should alert someone (I AM SICK)

A lot of the ones i can find online are the same, they don't actually flag (even visually) that you've given a bad answer. They just say "thanks for submitting" regardless

We really want it to direct you to a screen saying "You can't enter, please leave" or something for the security guard to identify, otherwise that person is already on our site.

Does anyone have any experience with this?? I can't believe it's so difficult to find one!

Selecting Yes for being unwell or being near someone unwell would direct them to a page that asks them to take an action (leave, contact a supervisor, etc). If they select No it would submit as normal.

    IIRC you could add an image or large coloured text to those pages for easy identification by staff when the screen is shown.

    Is this close to what you looking for? First search result from Google. Is this one that doesn't flag bad answers? Wouldn't as part of your work covidsafe plan to have a person checking the entries? I don't think you can transfer that responsibility to a computer screen under WHS Act, right? Someone has to be there to stop your workers coming in, no?

    You mean scanning the QR code on the Service NSW app for contact tracing, as you have essentially described?

    Just get them to yell shout, "TODAY, I AM NOT SICK" when they enter the office each day.

  • lol, people can just lie on it anyway, sounds a bit over the top to me, i'd tell you to shove your job. If people are going to work sick is your bigger problem, scared??

      True. A form isn't going to solve the problems in the world, it's definitely not going to keep you safe from the big bad scary wolf/virus.

      Let people work and let customers shop. If they're afraid of getting sick tbey can hoard up in the basement and stay safe.

    Maybe because the idea is dumb. If they answer they are sick, they have to go home? Why wouldn't they just take a sickie, instead of getting ready, drive to work, then go home.

    It's because this is the kind of thing only businesses want, so you have to pay for it.

    It's a super dumb idea though, as it doesn't actually stop sick people coming, just lets the boss pretend he did something. ("It's not our fault, the worker lied!") Useless. If you just want this kind of cheap deniability, have a sign saying "You MUST go home if you are sick". Better than having every worker waste 5 mins a day on this.

    If instead you actually want to prevent COVID in the workplace, have one of those things that scans people's temperature at the door, and hand out free face masks.

    • I'm not saying we won't pay for it, i'm just saying I can't find anything. Even from all of our existing providers (we have visitor check-in system, induction system, swipe card access system, thermographic camera system - none of them have this feature)

        (plus check-in, inductions, swipe card access… If you're paying for those separately, and replace them all with 3D Safety, might save you some money)

    • Do you know how much money a factory worker makes……….? I don't work at a McDonalds yknow, I work for a company worth $22b, there ain't no minimum wage people here

      And we're not going to sue them. Hell, i don't know what they're going to do with that information, it isn't really up to me

          Isn't it managerial instinct to cover ass and not necessary enhance safety or know why else you doing it?
