Go to PlayStation Store and select your profile at the top of the screen.
Select Redeem Code from the drop-down menu.
Enter the 12-digit code and select Continue.

It's weird how they're labeling this the PlayStation 15th Anniversary, as when that actually happened in 2010 they released this Avatar
Maybe they don't want to call it the *PlayStation Plus 15th Anniversary, which I guess is far less of a reason to celebrate given it eventuated in locking online play behind the pay wall.
