A duplexer allows automatic double sided printing.
They are the additional parts that can be useful for some people, or user replaceable parts that wear out.
e.g. Sheet feeder usually doubles paper printer holds so you don't need to fill it up as much. Not worth cost at home but may be worthwhile ina busy office.
The fuser unit is usually rated for ~20K pages but does wear out. Roller unit wears out ~50K pages.
Most cheap printers don't even allow you to change these, so you usually just buy a replacement printer.

Just as a comparison, I bought a C2100 (older model) in 2007 for around $750. The unit is slightly faster (25/20ppm) but uses older style toner cartridges that cost a mini fortune each.
This is a good deal for $287 if you don't need scanning/faxing etc.
Nearly $550 for toner cartridges.
