Steps:

Click on each of the videos to unlock a code for each theme/avatar pack

After watching the 1-2min video a redemption code will generate.

Keep in mind US accounts only, but once the theme is downloaded on the PS4 console, you can use it on any account.

  I kept getting that error because my Australia Sony ID had signed in automatically. When I signed out and logged in with my US account the rewards worked.
