Granpa: Hospital Social Worker, ACAT & Nursing Home

Granpa was discharged 2 weeks ago and safely arrived home. Thank you for all your advice and kind words.

Granpa was again admitted few days ago for Pneumonia, with the ACAT scheduled for an assessment this coming Friday.

I believe it was the hospital social worker's kind referral for this assessment to come so quick.

Whilst gramps being looked after by the hospital medical team, i still want the social side of it i.e. ACAT & Nursing Home to still be under way.

In our (family) opinion (after a long thoughts and discussion over discussion), nursing home is the most viable and best option for both Granpa and fam.

Apology for the long fill, but the question that I have, if anyone may experienced this in the past/recently even perhaps social worker:

  • For me to still have the Assessment done and push for nursing home solution, do I make the call to ACAT, hospital social worker or both?

I had a thought of having the assessment to relocate to the hospital however I'm not entirely sure if ACAT will assess 'not the best day' gramps.

  • Have you scheduled a family meeting at the hospital to discuss the options? This is the best starting point, not only to get the best result for Granpa, but to ensure all family members are on the same page and have a chance to ask questions of the social workers, OT, nurses etc and express any concerns or requirements etc.

    • yes, i will try to do that, once he settled in the ward.

        My wife is an OT and spends half her day in family meetings to discuss similar situations to what you described. It's super important to get everyone on the same page early so there's no arguments within the family on how to handle Granpa's health. Disagreements of this nature can lead to life long resentments. It means all the relevant family members get all of the medical advice from the horses mouth (medical staff) and not 3rd hand. Then the right decision can be made in best interests of Granpa.

        Contact the hospital and ask for a family meeting.

          we're a very small family; Granpa only has 1 daughter (my mom) and 2 grandsons. In the context of 'same page', all of us are in the same page and that includes gramp. it wasn't an easy decision however the most viable ones and sustainable.

  • so latest update is:

    • the hospital social worker rang and she's cancelling the ACAT assessment this week because they (ACAT) need to assess him at home and not in hospital.

    I guess it's a restart again from scratch; I need to call the ward where he's admitted and request his assigned (if there's as such) social worker for a quick meeting at the hospital?

    From the last forum, I understand that it would be significantly faster to have the hospital social work's referral, rather than me making that phone call to myAgedCare? is this accurate?

    • It really depends on the situation. Did you ask them when he is being discharged ? My aged care would've already received his referral and I don't think you need to request for another one unless they have cancelled the referral. I do hope that he will be approved for a low care respite at least.

        I believe this time will be a shorter stay than his first one but all they did so far was just run CT and X-Ray; no rectifying action done as yet. Just getting ready with his social affair, considering he's in the hospital because once he's out, I believe it'll be a reduced/lower priority for ACAT.
