Make sure you fill in ALL the fields - adddress and just above where it ask you for Which areas of health and wellness are you most interested in?' worked for me after a little fudging of the address

I put the street number in the first address line and the street name in the second address line and it seems to have worked.

When filling in the form, make sure you scroll down and check what the errors are when it appears that the form is not working. Every field has to be filled in, even if you feel it's not applicable.
I guess that's one way to make sure you don't have to give out too many samples…make sure every 2nd person can't submit the form.
