          thanks, it was just a bit sus the form was hosted on some public form site that anyone could've created

          publishing the source would've alleviated this concern in first place.

      You can host anything you want at JotForm, it's similar to Survey Monkey. I used them almost 6 years ago and thought they were great. Wouldn't judge them based on a single user's form.
