Looking at signing up to this card for my business. It says that you are excluded if already hold an amex in last 18 months. Does that mean a business amex or excluded if have you have a personal amex.
Next question, if you are supplementary holder on an AMEX does that also exclude you as holding an amex in the last 18 months?

generally business and personal are different in their minds so holding prior personal and now applying for business should be fine. I also assume you are asking due to sign up bonuses and if you are eligible or not.
Supplementary card holders are treated differently, it doesn't tie to their own application / card as you are the security for them. They simply check to make sure the supplementary person exists - there is no income or credit report checks if I recall correctly.
the 18mth counts all personal/business cards, as long as you're primary account holder. Supp holder doesn't count
Depends if you are the director of your company.
I have my personal amex and when I asked about the business version (still needs a name to go under), they said that the personal one counts
