The code is basically just $2.95 off for the premium surcharge, but you can get more than that with a $7.95 code + $2.95 surcharge or 40% off codes.
I've given up trying to work out Domino's codes.
EDIT: I know where I screwed up, the stupid menu now has a "My Menu" header which lists all previous pizzas I've added.

Just felt I should add that the free Loaded Supreme I received last week was in fact loaded with heaps n heaps of toppings. I would never pay the full price (since I could almost eat at a restaurant with napkins and "Can I help you?" for that price) but I was very impressed.
With all these Dominos deals I dont even have to say 'pizza for hell0' anymore, they know its me…
Also, has anyone seen the advertisment for Pizza Huts Hunger Healer pizza meal? It spruiks one (6-slice) pizza and a can of coke as being a great idea for a party and has 6 guys eating the one pizza and can of coke… worst party ever.

This code doesnt work properly. Common sense indicates that you should be able to get a premium pizza for the usual $7.95 price with the $2.95 surcharge removed, but thats not how it works. You get charged $13.95 for a $16.90 pizza. If you try to stack with the $7.95 coupon code, the $2.95 surcharge comes back and the "Free premium upgrade" doesnt work.
I had similar problems with the "Free bacon" and "free extra cheese" codes before.
Looks like you have to pay "traditional pizza" prices for premium, or $11.95 in my case
