Check your emails and see if you were included. The survey link appears to be unique for each email. Survey is pretty long-winded though, if you answer it properly it'll take about 3 minutes.

Once you complete the survey you will be able to choose between the two gift cards. If you choose a cash voucher, your $5 cash gift card will be sent to your email within 14 days. Cash gift voucher can be converted into the options found here. If you choose a Uber gift card it will be automatically applied to your account within 14 days and will expire on 31 December 2019. The information we collect will be treated in accordance with our privacy policy. Survey will be open until May 25 2019 11:59pm.
