Be aware that you need to order the sim online to get the $25 discount (not only the plan as the wording makes it sound like in the t&c). I bought a sim from a store and then activated and tried to buy the plan but it was $40. I contacted them on live chat and they gave me a $25 credit on my account in the end.

Yes. You would get. Though for Shopback, its only once per account. So, if you already got once earlier, you wont get now again.

Applies for Cashrewards as well. Although, check their T&C as I am not aware of them.
I hate 28 days plans. You pay one month extra every 12 months.
