When I entered there was no place to provide proof of purchase, I still received the spoon with no questions asked… I believe you can just enter your details and you will receive the spoon within a couple of weeks.

Update: Entry form has changed and now requires e-mail verification. The address must be entered by auto-complete (reload the form if auto-complete does not work).

    I never got mine, my parents did though. I got some email saying if you haven't received it or tracking info, contact support but now it requires uploading a valid receipt! Oh well.
