Sounds good but can we have some background info on the survey? Are you running it? Or are you linking to another companies survey? Survey seems fairly painless, though!
From what i understand surveymonkey site is a member site (i may be wrong) so how can this be a legit offer without joining??
Worked out that you need to fill in all the boxes in the Name/Address/Email part. Whether or not you make up an email and phone number is up to you to prevent them contacting you…

Looking at the surveymonkey website, free surveys are limited to 100 responses.
There are 100 free SD cards.
No company is mentioned
Any country is allowed
Did someone just make this to obtain some personal details?
Seems to be scam to collect personal information. Various tricks are being used these days to collect as much personal information as possible. I prefer to skip such surveys when no company information available.

Its not very useful information, especially when all you need to do to find that info is look in a phone book.

I am yet to receive anything for completing a survey, especially ones posted on this forum……hence voting -ve. Happy to withraw it once someone actually receives their card…….
Is a 2gb sd card really worth the time spent and putting your details at risk of on-selling? How much are they worth? $2?

In answer to the questions:
I designed the survey, anyone can create one through surveymonkey.com. Yes there is a limit to 100 responses for free surveys, however I am a paid member so that limit does not apply. I received far more responses than I have SD cards, and this was not the only forum I sought a response. I have no way of knowing if the first 100 are a few or a lot of Ozbargainers but rest assured a number will receive cards. I apologise if you do not receive one, however the response was far greater than I had hoped. To thank everyone for their time, once the store is up and running I will offers some Oz Bargain exclusive deals.
I also thought that I stated in the survey that email addresses and phone numbers were not required, and many people did not submit them. If you did however I will not be using them in any way.
Looking froward to seeing the store and hope the answers were useful :)

I'm a bit concerned that by leaving the non-essential field empty means I didn't "complete" it properly. Guess I will know by whether I actually receive on in the next couple of days.

I did them manually so if there was a name and address and you were in the first 100 you would have been sent one. They were all sent Friday so you will know in the next day or two.
