I believe you freeze the bowl first then add your ice cream mixture. Attach it to the kitchen aid and then leave it churning for 20 mins while the cold bowl freezes it.
You have to freeze the bowl for something like 4-8 hours in the freezer before using - the bowl itself is more like a thermos flask, but instead of a vacuum, is full of liquid similar to the plastic freeze blocks you can get for the kids drink bottles.

Do be so sure - there are two versions - one for the 120V USA machines, and one for 240V Euro/Asian machines.

I bought my bowl from the US and Kitchenaid locally and have no problems. I do have the Proline version of the Kitchenaid and also have the ring too.

I don't know - I don't believe they have an online store, but give them a call and see if they'll ship it.
Depending on where you are Gumtree might be a good option. Theres always been a couple of brand new ones floating on gumtree whenever I have checked. Normally asking $100-$130 or so for a new in box one. Could be cheaper that getting it posted.
Yes they ship them interstate - it was $20 for shipping to Sydney. Apparently they are getting Ozbargained, when I called and asked if they ship interstate they straight away asked if I was calling about the KitchenAid Ice Cream Bowl! Also they won't accept the sign-up voucher, as it only applies to non-sale items.
